2. For Change Due To Incapacitation Or Disability
In-Person – Signed conservatorship or power of attorney and valid government-issued Photo ID. If Home Security is on account, the request should be mailed or faxed.
By Fax/Mail – Signed power of attorney or conservatorship and a Notarized Form.
3. For Current Customer Being Deceased
In-Person – A death Certificate is required and valid government-issued Photo ID. If Home Security is on account, the request form must be mailed or faxed.
By Fax/Mail – Death Certificate is required and should be Notarized Form
Give Spectrum at least a few days for the account transfer. You may follow up using the reference number you were provided for the transaction.
Check your bill next month to find out if the service has been transferred to the new owner.
